1. Please fill out the relevant insurance form and enter either the date you have been issued a German visa or the date of arrival in Germany as the date that the contract should commence.
2. Please fax the signed application to the fax number given in the form. If you don’t have a fax, then please scan the document and send it to the GASP Insurance Service

Please calculate this time when going to pick-up your visa!!
